Which artist? is always the first question: any available artist, or one of your artists. It's fixed.
Drag the other questions to reorder, show or hide them, and click one to edit it. Add question adds your own.
Click Preview to open the real form in a new tab.
Good to know
Requests sent to one of your artists directly use that artist's own forms.
