What you can do
Consent forms at your studios (only if a studio you work at has its own form): shows which studios use their own form and which let you use yours.
Send automatically: Bookpay sends the form 48 hours before the appointment. If you turn it off, show a QR code at the session for the client to scan and sign.
Studio market: sets the legal sections and the languages of your form.
Studio: printed on every signed record (Change opens Account > Studio), and your local registration / licence no. (optional).
Age requirement: 18 years or older. Clients confirm their date of birth online and you check photo ID in person. This can't be turned off.
Allow younger clients with guardian consent: your decision. Check your local law first. Bookpay hasn't verified it for your market.
Form language: English and the local language. Offer both languages to clients adds a language toggle (only when both translations are complete).
Health conditions, Risks & consent and Aftercare: drag to reorder, Add your own, or Reset.
Data & photo consent: separate health information consent and a data retention notice (both required), and asking to use tattoo photos in your portfolio (an opt-in, unchecked by default).
Click Preview, then Save changes.
Good to know
Your form is used everywhere a studio's own form doesn't apply: your own space, other studios, and guest spots where the studio has no form.
